The shoulder is the most flexible joint in the body, allowing extensive movement. However, this mobility also makes it inherently less stable and more prone to injury, making it the joint most often dislocated.

 

Pain in the shoulder can either be localised or felt in nearby areas, such as the arm or the upper back.

Causes

Arthritis

  • Osteoarthritis: Degeneration of cartilage in the shoulder joint leads to pain, stiffness, and a reduced range of motion.

  • Rheumatoid arthritis: An autoimmune condition that can cause inflammation and pain in the shoulder.

Heart Conditions

Cardiac conditions, such as angina or a heart attack, often cause referred pain in the shoulder, particularly the left side. This occurs due to the nerves transmitting pain signals from the heart also connecting to the shoulder.

Organs

Pain in the shoulder can sometimes be referred from internal organs due to shared nerve pathways. For instance:

  • Gallbladder disease or gallstones can cause referred pain to the right shoulder.

  • Lung issues, like a tumour or infection, may cause pain felt in the shoulder area.

  • Heart problems, such as a heart attack, can cause referred pain in the left shoulder.

Neck (Cervical Spine)

Bursitis

Issues in the cervical spine, such as herniated discs, arthritis, or nerve impingement, can radiate pain to the shoulder. The shoulder and neck often share nerve pathways, causing overlapping symptoms.

Also known as adhesive capsulitis, this condition occurs when the connective tissue around the shoulder joint thickens and tightens, leading to stiffness and pain that worsens over time.

Bursitis refers to inflammation of the bursae (small, fluid-filled sacs that reduce friction in the joint). Shoulder bursitis can cause localised pain, especially when moving the arm.

Tendonitis

Fractures

Tendonitis is inflammation or irritation of the shoulder tendons, often caused by repetitive motions or overuse, leading to pain and restricted movement.

Fractures of the collarbone, shoulder blade, or upper arm bone typically result from trauma (like falls or accidents) and cause sharp pain, swelling, and limited mobility.
